Physical activity improves quality of life and reduces hospitalizations in heart failure (HF) patients, but many engage insufficiently. Existing strategies often overlook HF patients’ unique, time-varying biological and psychosocial factors or are resource-intensive, highlighting the need for innovative approaches. This proposal aims to: (1) characterize real-time factors affecting physical activity in a varied HF cohort using mHealth and ecological momentary assessments (EMAs) guided by self-determination theory; (2) adapt an existing mHealth intervention via user-centered design to optimize notification content, tone, frequency, and contextual tailoring; and (3) pilot a 12-week just-in-time adaptive intervention (JITAI) in 30 HF patients, delivering context-tailored notifications via smartwatch to increase physical activity, measured by step count within 60 minutes post-notification.
This SP14 is interested in the personalization algorithms proposed by TR&D2, particularly how causal domain expertise can be utilized to improve personalization. Further, this SP aims to use the (under development) JusTIn Toolkit for Just-in-Time Adaptive mHealth Interventions and the pJITAI toolbox. The mDOT Center provides novel intervention content using Generative AI and stress monitoring to SP14. With highly personalized mHealth interventions at the right time, physical activity recommendations will become more responsive to HF patients’ needs and contextual factors.
